Hi Holmfirth, sorry for the delay in response

Application notice form n244

http://www.hmcourts-service.gov.uk/courtfinder/forms/n244_e.pdf

Along with an accompanying affidavit attesting to the facts of the matter as you know them and including the main issues to be heard and the relevant law on the matter.

There is a fee, £75 i believe (unless you are eligible for a fee remission)

Mr Frodo, That's exactly what I've been after - thank you SO much!
I actually posted the latest communication (`Further Steps Notice') back to them today as `Refused for Cause' as I was unsure what to do. Do you think I could apply for a judgement in tandem with this? Any help gratefully received, thanks again.

Sorry for the delay in reply,

The possible strategy is to put in an application to set aside the judgement under Civil Proceedure Rule 3.4 (2) (check the rules for which part of that section applies). You entitle your affidavit something along those lines eg, Application to set aside judgement under CPR 3.4 and you use your affidavit to set out the facts of the matter, the relevant law of the matter and any case law and legal authorities you intend to rely on (though these last 2 can be left to the skeleton argument should you write one).
